Informed Consent for Massage Therapy
Last updated September 17, 2026
You accept this form when you book. It explains what massage therapy involves, what it does not, the risks, and what you and your therapist each agree to. Please read it before your session.
Consent to treatment
By accepting this form, you confirm that:
- You voluntarily request and consent to receiving massage therapy.
- You do not currently have a contagious condition that could put your therapist or other clients at risk.
- You understand the massage offered is for the therapeutic purposes of general wellness, stress reduction, and relief of muscular tension.
- The nature of massage therapy — its potential benefits, effects, risks, contraindications, and alternative therapies — has been explained to you, and you understand that information.
- You have had the opportunity to ask questions, and your questions have been answered to your satisfaction.
Risks you are accepting
Massage is generally safe, but it is hands-on work and carries some risk. Those risks include, but are not limited to:
- Superficial bruising
- Short-term muscle soreness
- Aggravation of an injury you did not know you had
There may be additional risks specific to your physical condition, which is why disclosing your health history matters.
What massage therapy is not
Massage therapy is not a substitute for medical care, examination, or treatment. Your massage therapist does not diagnose illness, injury, or disease, and does not prescribe medication. Nothing said during a session should be taken as medical advice or a diagnosis.
For any physical or mental health concern you are aware of, please consult a physician or other qualified health provider.
Your responsibilities
- Tell your therapist about your health. Disclose all relevant medical conditions and any medications you are taking, and let them know if anything changes before a later session.
- Speak up during the session. If you feel pain or discomfort, tell your therapist immediately so pressure or technique can be adjusted. They cannot adjust what they do not know about.
Professional boundaries
You will be properly draped at all times. Only the area being worked on is undraped, and you may remain clothed to whatever degree you are comfortable with. You will never be asked to expose any part of your body you do not wish to.
Either you or your therapist may end the session at any time, for any reason. You do not need to justify that decision.
Zero tolerance for harassment
This is a professional therapeutic practice. Any sexual remark, request, or contact — verbal or physical — will end the session immediately. You will be charged the full price of the session, you will be permanently banned from booking, and a fee of $350 may be charged in addition. Conduct of this kind may also be reported to law enforcement.
The same standard applies to therapists. If a therapist behaves this way toward you, tell us and their access to the platform ends immediately.
Cancellations
Please give at least 48 hours notice to cancel or reschedule. Cancellations inside that window forfeit your deposit, because your therapist has already paid for that treatment time and can no longer sell it. Full details are in our Terms of Service.
Release of liability
You agree not to hold your massage therapist or Universal Health & Fitness, LLC responsible for ordinary soreness, bruising, or discomfort that results from massage performed with reasonable care, and you release them from claims arising from such ordinary effects of treatment.
This release does not cover gross negligence, reckless or intentional misconduct, or anything that cannot be released under Utah law. It does not limit your right to raise a concern about the care you received.
Withdrawing consent
Your consent is informed and voluntary, and you may withdraw it at any time — except in respect of services already provided or actions already taken. Withdrawing consent means ending the session, and you can do that without explanation.
How your acceptance is recorded
You accept this form when you book. We record which version you accepted and the date and time you accepted it, so there is always a clear record of what you agreed to. This is version 2026-09-01. If we change this form materially, you will be asked to accept the new version before your next booking.
